All the prize winning tickets above Rs.5000/-
should reach the office of Director, Punjab State Lotteries, SCO
106,107 & 108, Batra Building, Sector 17D, Chandigarh 160 017 with
the
Prize Claim Form within 30 days from the
date of publication of the result in the Punjab State Government
Gazette either personally or through Nationalised Bank or by
registered Post or by insured parcel.

In case the prize winner's claim does not
reach this Directorate within 30 days the right of the winner to
claim the prize amount thereafter shall be extinguished.

Prizes upto Rs.5000 should be claimed from
the sellers or Agents within 30 days from the date of
publication of the result in the Punjab State Government
Gazette.

Instructions

Affix three passport size Photographs
with Claim Form out of which two should be duly attested by 1st
Class Magistrate/ Notary Public/ Oath Commissioner or any Gazetted
Officers.

If
the Claimant is Minor then an Affidavit from the Natural
Guardian/ Parent should be attached with the Claim Form.

If the claim is not claimed within the prescribed period (30
days from the date of Gazette Notification) then reasons of this
on an Affidavit should be attached with the Claim Form
